Cost & Fees

Financing therapy is often a concern for clients. For your convenience and to ensure that our services are available to the most people possible, we offer several options for handling the cost of your therapy.

Insurance benefits for outpatient behavioral health services vary from insurance to insurance and plan to plan. The best way to find out if the services are covered by your insurance and if you have a patient responsibility is to contact your insurance directly. When calling your insurance to check benefits or conducting an online search for us, your specific therapist’s name may or may not be individually affiliated with the insurance company. Some insurances recognize us as a group. It is often helpful to use the Chosen, PLLC National Provider Identifier (NPI) 1194120097 or our Federal Tax-ID 90-1034222. Your insurance company is able to easily search for us with these numbers.

See profile of specific provider for questions to ask your insurance company before your first appointment.

Here is a link to an article that you may find helpful in understanding your insurance information. While the document specifically references BCBS patients, the information and language is general enough to apply to most insurance plans.

If there are any additional concerns you may contact Dr. Brooks or the specific provider you are interested in working with to obtain clarification. As we are always expanding our reach, this is not an exhaustive list of the insurances we accept. You may also have out-of-network benefits coverage associated with your plan which may be an additional option.
