Financing therapy is often a concern for clients. For your convenience and to ensure that our services are available to the most people possible, we offer several options for handling the cost of your therapy.
Insurance
Insurance benefits for outpatient behavioral health services vary from insurance to insurance and plan to plan. The best way to find out if the services are covered by your insurance and if you have a patient responsibility is to contact your insurance directly. When calling your insurance to check benefits or conducting an online search for us, your specific therapist’s name may or may not be individually affiliated with the insurance company. Some insurances recognize us as a group. It is often helpful to use the Chosen, PLLC National Provider Identifier (NPI) 1194120097 or our Federal Tax-ID 90-1034222. Your insurance company is able to easily search for us with these numbers.

Good Faith Estimate
You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost. Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for the medical items and services. You may request a Good Faith Estimate at any time, additionally, one will automatically be provided to you in accordance with the law.
Private Pay
Clients who do not have insurance are able to pay for services directly. The following costs apply:
Initial Intake $250
Follow-up session $200
Late Cancellation/No Show Policy
Once an appointment hour is scheduled, you are asked to provide a 24 hours’ notice of cancellation or you will be charged for the appointment. Please contact one of our therapists to discuss individual options. Your insurance policy will not cover this fee.
